Summary of Responsibilities:
Under the general direction of the Family Finding & IL Supervisor, this person will work to engage extended family members in service and involvement with the child and family. This position will perform activities on behalf and in support of the Family Finding Program including promotion of the Family Finding Program with Case Management, referred families and the community. Duties are conducted in accordance with all existing policies, procedures and regulations and in accord with the Mission and Values of the Company. This position has been designated to be a part of the County’s CYS workforce for purposes exclusive to the proper handling of consumer information as is required by SAM privacy policies.
Essential Functions:
Maintains case records
Meets with families to set up meetings
Assists/facilitates family meetings
Prepares documents
Provides transportation to meetings
Creates eco maps with families and youth
Assists families in building their family tree
Assists with required referrals
Develops plans
Engage families during meetings and processes
Prepares Family Finding documentation
Maintains the Family Finding file
Assists with paperwork to access services
Plans/organizes/leads family finding meetings
Collects and reports data
Attends required trainings
Prepares court documentation and testimony
Ensures referral source needs are being met

Education and Experience:
Bachelor’s Degree supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or two years experience as a paraprofessional in similar position and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience in a similar position performing case management functions.
Employment Requirements:
Automobile and Valid Insurance, Valid Pennsylvania Driver’s License and Acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance
Working Conditions:
Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, and travel throughout the assigned region may occur and may be in adverse weather conditions, must be able to lift up to 15 pounds.
